S2960 ports problem to issue the IP Address

I have a stack 24 ports and 48 ports using the stackable module. The stack is configured to do the DHCP Server Port-Based Address Allocation which will assign a fix address to each port. I have problem right now is could not get the ip address from port 12 and 14 same on both switches. I also have and another stack that build with the sam confgiuration and has the same problem: port 12 and 14 on both switches could not get the ip address. Any idea that what causing this problem. Attached is my config file and here is the version:
