Generally you will get this error when you are upgrading the IOS on Cisco 2851 router because it is failing to copy an IOS image can be attributed to a bad or corrupt flash disk. Try the bug id CSCsb42671& CSCsa54996. To fix this bug, I would recommend that you upgrade to the latest of the 12.3(11)T release which is 12.3(11)T8.
