VoIP configuration for SRP 527W

Hi

I recently purchased a 527w and need advise for setting up my existing VoIP service.

The Following are the only details I have previously used with other makes of router but I cant get the CISCO to register with the service so I guess I'm not completing the correct cells in the web interface.

I have 2 numbers with details as follows

Obviously I have @@$$ my password and subscription details, so which cells do I put the above info into? I tried this and several other combinations (again the below id the actual info I put in the cells)

I tried what I thought were the correct ones as follows
